IHP recommends attendance of symposium, "Better Upstream - Better Downstream Care."
IHP is very excited to be part of an upcoming symposium on social determinants of health titled Better Upstream - Better Downstream Care. This all-day session will be held on the Oakland University campus in Rochester, Michigan on Wednesday, April 18, 2018. It will convene health care leaders, social service agencies, academics, payers, employers, and elected officials from Michigan and Canada to raise awareness of the critical need to address the social determinants of health to increase positive health outcomes for all of our citizens.
Acceptances to speak at the symposium have been received from two prominent national health care professionals: Rishi Manchanda, MD, MPH, President and Founder, Health Begins, and an “upstreamist” activist and change agent and Paul Grundy MD, MPH, FACOEM, FACPM, IBM’s Global Director of Healthcare Transformation and the "godfather" of the Patient Centered Medical Home.
Because this opportunity supports the practices’ and IHP’s work on social determinants of health, we are canceling the Care Management Collaborative scheduled for that date. Please mark this date on your calendar and plan to attend. As more information is available, we will share it with you.
