Integrated Health Partners would like to invite you to join us in celebrating our 20 years of success in Battle Creek, MI. IHP has cultivated a culture built on our mission, vision and values.

IHP proudly partners with Bronson Battle Creek and the Calhoun County Physicians Organization. Over the past 20 years, our membership has almost doubled to 164 members. We started offering 3 services and have grown to offer 10 services to our members and the community. We currently offer Credentialing, Contracting, Utilization Management, Care Management, Condition Management, Practice Coaching, Quality Improvement, Data Analysis, IT and an independent insurance agency.

Milestones

1995 - Business incorporated in August and first staff hired the following January

1996 - First health plan contract executed

1998 - Assumed new dba name of Integrated Health Partners to be               more identifiable to the community and with our mission

1998 - First delegation agreement for credentialing/recredentialing and           utilization management

1999 - NCQA Certification for Utilization Management and                             Credentialing and Recredentialing

2003 - Formation of Partners Insurance Agency of Southwest                       Michigan, Inc.

2003 - 2008 - Offered group health insurance to physician offices

2006 - IHP patient care registry created

2007 - First IHP Learning Collaborative
            Then: 10 office teams
            Now: 35 participants from 18 offices in the MA collaborative
2009 - First IHP offices designated Patient Centered Medical Home
            Then: 5 practices out of 25
            Now: 24 practices out of 25

2009 - Partnership with Small Business Association of Michigan

2012 - MIPCT Care Management Program started

2013 - Condition Management Program started

2014 - Relocated from Fieldstone Center to Commerce Pointe in                   Downtown Battle Creek
