Practice Performance Improvement Support

IHP provides each physician practice with a dedicated Practice Coach who is Health Care LEAN certified and skilled to help practices reach their performance improvement goals. Coaches share best practices, analyze practice performance data, assess practice competencies to excel in quality improvement efforts, and implement Patient Centered Medical Home – Neighbor (PCMH-N) practice transformation concepts.  Several practices have a dedicated Clinical Quality Coordinator (CQC) who provides additional support with quality and process improvement.  The CQC works alongside practice staff to help them develop efficient and effective processes.

Health Plan Incentive Program Support

Practice Coaches and a Clinical Quality Coordinator work with physician practices to evaluate health plan incentive programs, clinical quality measure specifications, performance levels, claims data, and opportunities for improvement.  A Quality Improvement Coordinator (QIC) supports quality improvement initiatives including practice coaching, data management and reporting, and other value-based reimbursement programs. 

Educational Support

IHP offers multiple educational methods and learning opportunities for patients, providers, managers, and office staff.  Each method is designed to enhance the understanding of evidence-based care guidelines, coordinated care, and collaborative relationships.  These educational opportunities are ongoing and increase patients’ ability to self-manage their quality of life and chronic conditions.  Education for physicians and practice staff also assists the entire medical office staff work more effectively as a team.

Provider Services Support

A Provider Services Coordinator assists practices by leading, coordinating, and supporting activities related to provider and facility onboarding, credentialing, and contracting.  The coordinator is the primary liaison between providers and health plans for provider participation, routine updates, and claims questions along with being the expert in credentialing, recredentialing, and health plan contracting.

Credentialing & Recredentialing Services

  • Certified by the National Committee for Quality Assurance (NCQA)
  • Required for physicians, advanced practice professionals, and other providers
  • Includes obtaining, verifying, and evaluating credentials and primary source verifications 

Health Plan Contracting Services

  • Competitive health plan reimbursement rates, incentive programs, and contracts
  • Contract education, periodic reviews, rate negotiations, ongoing relationship maintenance throughout life of contract, and reviewing contract issues as needed
