Objectives: Review program updates, resources, and data to support patient engagement in care management. Review evidence-based guidelines related to HEDIS measures, including Transition of Care (TCM), and review practice level data. Discuss best practices to improve outcomes. Provide an overview of BCBM behavioral health care management program. Identify opportunities to collaborate with PCP practice care managers to improve patient outcomes through a cohesive team. Provide suicide prevention training and resources to assist Care Managers working with patients having a mental health crisis. This event will be held at the IHP office. A snack and lunch will be provided. Target Audience: The intended audience for the meeting is care managers. Practice managers and billers are encouraged to participate in program update segment of the training.
